Violent Incident Reporting Form

The Violent Incident Flow Chart is here to guide you through what to do if you ever experience violence at work. Violence doesn’t just mean physical harm, it also includes threats or actions that make you feel at risk.

This chart walks you through the steps to take right away: making sure you’re safe, getting medical help if you need it, letting your supervisor/principal know, and recording what happened while it’s still fresh. It also reminds you to note witnesses and follow up with a WCB claim if you’ve been injured, either physically or psychologically.

Having this process laid out is important because it takes the guesswork out of a stressful situation. It makes sure nothing gets overlooked, that you get the care and support you need, and that incidents are properly reported so we can all work toward safer schools and workplaces.

By following the chart, you’ll know exactly what to do and can feel confident that you’re supported every step of the way and remember, your Union is here to support you too if you need help navigating the process.
